Forms

Many legal proceedings involve the use of forms, which may or may not be available directly from the court. This page will give some suggestions for places to look on the internet and at Jenkins Law Library to find a selection of sample forms. Please note, many of the forms in these resources are only examples. Most sample forms from any resource will need to be modified or rewritten to fit your specific situation.

Jenkins Law Library is open to the public for a fee of $5 per day. Print charges are $0.25 per page and copy charges are $0.10 per page. You may conduct your own legal research in our collection. Internet computers with database access may be used for 1 hour per day. Word processing computers (no internet) and a typewriter are also available for extended use. A reference librarian is available to assist with the collection and computers, but cannot advise you on legal matters.

Librarians and staff at Jenkins Law Library are not able to give legal advice, select or fill out forms, write briefs or memorandum, or conduct any research for you. Includes forms and information regarding child custody, child support, separation and divorce, helping an abused child, and adoption. These forms may also be available through other websites.

To find interactive forms, search database for "LawHelp Interactive" and select "Online Resources, Self-Help Resources and Forms" from the left side panel. Pennsylvania Child Support Program Department of Human Services
Includes information and select forms for child support in Pennsylvania. Also links to the Pennsylvania Child Support Handbook, which also contains county Domestic Relations Sections telephone numbers.

Online - Interactive Family Forms from Phila Legal Assistance

Philadelphia Legal Assistance has developed seven online document assembly forms for pro se litigants in Pennsylvania family courts. These forms were developed with funding from a Legal Services Corporation Technology Initiative Grant and are available now on PALawHELP.org.

Judicial Conduct and Disability Administrative Office of the U.S. Courts
Congress has created a procedure that allows any person to file a complaint in the courts about the behavior of a federal judge, but not about the decisions federal judges make in deciding cases. This site links to each federal Circuit Court site containing the rules explaining what may be complained about, who may be complained about, where to file a complaint, and how the complaint will be processed.
